Where I work we do the following- a taped report with just the very basics. The reason we went to this was because of all the OT invloved with nursing reprt. Alot of info was being spoken about that wasn't needed to deliver care. Things such as BM's, meal intakes ect. Since all these things are documented somewhere else, we decided that it would be up to the nurse/CNA to get these facts if they so thought they needed them.
Our taped reports last about 3 minutes for 16 residents- and only pertinent info is shared.
Things such as fevers, vomitting, illnesses are reported. VS are not reported unless they are abnormal.
We also use a communication book for non-nursing issues such as maintenence needs, medical trips,
diet changes ect.
It is our policy that no one can deliver any care
until they have heard report and read the communication book, and this includes CNA's. It takes about 5 minutes at the start of shift.
