The progrom included alot of quizes and a final, what I thought was the state test. On May 30 of 02 I received a grade report of the final that statet, that I passed with a AB 3.00 and 10.50 points. Course Title: Basic Nursing Asst. Then we had 4 hours of training in a Nursinghome and a girl from our class applied for a job there and got hired. I received a small card in the mail from the Department of Health and Family Service Division of Supportive living Wis. Nurse Aide Directory with my name and SSN and inclusion date:05/29/02( I'm not sure exactly what it is). It does say on there, call the Wis. Nurse Aid Directory interective Voice Response at 1(877)573-1081 to obtain current nurse aide eligibility information. Failure to report your employment in a nursing or a nursing related capacity every 24 months from your inclusion date may affect your eligibility level.
I did call the number several times and for many days but at the time I always ended up with a busy signal. To this day I don't know what document it is. In another letter it says, we have printed a diploma for you indicating your sucess in this program, however I never sent the fee and never received the diploma. I'm still confused.
