YOu will need to talk to your state registry asap. You did not tell me what your state is but you can find this info on this web site.
Go to the buttons on the left side of this page Click on "New NA FAQ" Then click on "how to find a training program"
You will see a list of the States Nurse Aide Training Programs offices. Scroll down to find your state and call them.
Be ready to fax a copy of your SS, your current approval card, and verification of your employment this past 2 years. this can be a memo from your adminsitrator or DON or in some states they will accept your paystub.
You also may be able to get reapproval over the internet. Again, check the listing of State NATPs and if they have a web site listed, then go to this for more instructions.
