Dear Marian

The most fequent reason for losing your state approval is that they do not know you worked in the past 2 years. In some states this the verification of your work record is done by your employer. In other states, you have to do your own verification to the Board. You need to find out first if you are still on the registry, then if not, find out why and the steps to fix. You can do this and it will work if your problem is about records.

These are the steps to take
1. Gather these following info in one place - your social security card, the last approval card you got from your State, the dates of your training and testing and your most recent pay stub from working as a NA.
2. Look on the top of this page for a link that says " how to find a training program in your area" Open this and you will find a list of the State Offices for NAs. Look for your States office
3. Call them - be prepared to give them the info in #1 - ask them what your status is and how you can get reinstated.
4. Send or fax them the info they request
5. If you have problems with this, please email us privately at and we will walk you through the steps or we may contact the agency on your behalf.
