I believe that OR uses ASI as their testing service and the info and sample questions are suppose to be given to each NA while she/he is in the NA training program. I do not have this on my computer, but will fax it to you, if you can provide a fax number. As an alternative, you can call the state testing office and ask them to send you this info.
The address I have is about 1 1/2 year old so I am not sure if this is up to date, but you can call information for their phone number and call them:

We receive so many requests for this type of info that we are considering adding a page with the info for all of the states.
