Good to hear from you. Would love to know more about what you are doing in Holland? Do you have nursing homes? Medicare? How do you care for ill elderly and those who need long term care/

We have formal discharge agreements between hospitals and nursing homes. Each agreement specifies what people can be transfered, how this will happen, at what times, and what info has to go with the person being transfered.
There are several sites that may be helpful to you but I will have to go to my address book and will try to email you with these addresses.
