Depends on how long since you worked and if you are on the registry.
I suggest contact the Registry in Tennessee
Call the director, Jacqueline Johnson at (615) 532-5171. You may also want to try the toll free number first (888)310-4650.
When you call, be prepared to give them your SS number, the date you completed your training and tesing and the name and address of the last place you worked.
